Documents to bring to your appointment

After you booked your vaccination appointment, the following documents need to be filled in, printed on paper and signed. Please bring them to your vaccination appointment.

1.+ 2 Information sheet & medical history

The Information sheet (pp.1-6) contains detailed information on the vaccine, its effectiveness and possible side effects. The Anamnese sheet (pp.7-9) asks about the current state of your health. Please print the document, fill in both parts and bring them to your appointment. If you require the document in a language other than English, please visit the website of the RKI. 2. Anamnese

Das Formular der Anamnese fragt danach, wie gesund Sie im Moment sind, ob Sie regelmäßig Medikamente nehmen, die vielleicht mit der Impfung interferieren, etc. Bitte drucken Sie das Formular und füllen Sie es aus. Geben Sie alle akuten oder chronischen Krankheiten, Medikamente und Allergien an. Unterschreiben Sie das Formular und bringen Sie es zu Ihrem Impftermin mit.

3. Check-in form vaccination certificate

Before your appointment, please fill in the check-in form and then print (1) "Laufzettel" (step-by-step documentation). You also have the chance to fill in the (2) Anamnese here and print it. You can also print the (3) vaccination certificate which the vaccination centre will stamp for you.

4. Checklist

This checklist contains all the steps you need to take for an vaccination appointment and also details which documents and things you need to bring besides the above (e.g. vaccination ID, if you have one).
